Batchelor Middle School

The physical form must be completed on or after April 1, 2026 to participate in any sporting activity for the 2026-2027 school year. Students will not be allowed to practice/play until the entire form is filled out. This form needs to be uploaded and submitted on FinalForms (https://monroecounty-in.finalforms.com).
